Changing your Provider

You have the right to change your Home Care Package Provider for any reason at all and your current provider must comply with your wishes. Note that while some providers may charge you an entry fee or exit fee, Aston Home Care does not charge these fees, so you can bring your package to us with confidence!

If you wish to change from another Home Care Package Provider to us, you do not need to worry about any of the paperwork. Just give us a call and one of our friendly experts will meet with you to discuss your objectives and needs, and we can handle all the red tape for you.

If you don’t want us to do it for you and would prefer to do the changeover yourself, there are four steps required:

  1. You must notify your current provider that you are transferring your package and the date the transfer is to take effect. A minimum of two weeks’ notice is required but we recommend you allow three or four weeks.
  2. Your new Provider can only begin services once they have received a referral code from MyAgedCare. You must notify MyAgedCare that you are transferring your package to Aston Home Care and ask them to transfer your unique referral code to us. You can telephone MyAgedCare on 1800 200 422.
  3. Once you have received your referral code from MyAgedCare you need to inform us of your code and the termination date agreed with your former provider. You can call us on 1800 427 866 or send us an email to info@astoncare.com.au
  4. We will arrange to have your Home Care Package transferred to us. Our friendly expert staff will meet with you to discuss your objectives and needs. We will work with you to create your Care Plan and give you a new Home Care Package Agreement that you need to register with MyAgedCare. We can then start the services that will assist you to stay living in your own home with Dignity, Confidence, and Independence.

If you are currently with Aston Home Care and are considering transferring your package to another provider, you certainly have that right and don’t have to explain why. However, we would very much like to find out what prompted the change so we can make sure we fix any problems that may exist. We are very proud of our service and our caregivers and want to make sure we are not only meeting but exceeding expectations. Before deciding to leave us, you should ask the other provider the following questions:

  1. Do you charge an Entry or Set-Up Fee?
  2. Do you charge an Exit Fee?
  3. Do you charge a Basic Daily Fee?
  4. What is your Case Management Fee?
  5. What is your Package Management Fee?
  6. What is your Hourly Service Charge?
